• Report of Receipts and Expenditures for Constituent-Service Program (OCF Form 10)
• Withdrawal of Position of Treasurer for Constituent-Service Program (OCF Form 9)
• Acceptance of Chairperson for a Constituent-Service Program (OCF Form 11)
• Statement of Withdrawal of Position of Chairperson for a Constituent-Service Program (OCF Form 29)

Once completed, mail the form(s) to the Office of Campaign Finance (OCF).
